Dr. A H M Delwar is an Associate Professor of Otolaryngology at Comilla Medical College in Cumilla, Bangladesh. With a deep commitment to advancing ear, nose, and throat (ENT) care, he has contributed significantly to the understanding of vocal cord disorders, branchial cleft anomalies, and other otolaryngological conditions. His research portfolio includes over 25 publications, with a particular focus on vocal cord paralysis, polyps, and adenoidectomy procedures. Dr. Delwar holds an MBBS, MCPS in Otolaryngology, and is pursuing a PhD in Otolaryngology and Head-Neck Surgery from Chittagong University. Appraisal of Thyroidectomy in Outlying Tertiary Care Hospital
Objective: To find out the prevalence, presentation, disease patterns, operations of the thyroid gland and facilitate the prevention of complications and a risk factors for it. Study design: Cohort retrospective study. Setting: Academic tertiary care hospital. Subject and Methods: A total of 173 euthyroid benign and malignant goiter patient’s demographic data, diagnosis, operations, complications, and management options collection and analyzed who attended in the department of Otolaryngology and Head-Neck Surgery, Comilla Medical College, and Comilla Medical Centre, concern Clinic of Central Medical College, Comilla, Bangladesh from 01 July 2016 to 31 June 2019. Results: Incidence of euthyroid benign and malignant Goiter among outpatient was 0.12%, and the yearly prevalence of 33.34%. Out of 173, the male was 20 (11.56%), and the female 153 (88.44%), 30-39 years patients have the highest presentation was 60 (88.44%), 20-29 years 40 (23.12%), and 40-49 years 38 (21.97%), euthyroid benign goiter was 142 (82.08%), and malignant 31 (17.92%).
Spongstan with Nasopharyngeal Pack: New Ordinary Procedure Manage the Adenoidectomy Bleeding
Objective: To find out the frequency, prevalence, and epidemiological aspects of the adenoidectomy bleeding and share the new ordinary procedure to manage it by spongstan with the nasopharyngeal pack. Study Design: Cohort retrospective study. Setting: Academic tertiary care hospitals. Subject and Methods: A total of 23 adenoidectomy children’s demographic data collection and analyzed who suffered from complications of bleeding in the department of Otolaryngology and Head-Neck Surgery, Comilla Medical College Hospital, and Comilla Medical Centre, concerned Clinic of Central Medical College from 01 July 2016 to 31 June 2019. Results: Incidence of adenoidectomy bleeding among total operative patients was 0.32%, adenoidectomy-Tonsillectomy patients 2.66%, and the yearly prevalence of 33.35%. Off them, the male was 09 (39.17%), and the female 14 (60.87%), 11-15 years children have highest bleeding complications was 17 (73.91%), commonest presenting features was nasal obstruction (91.30%), mouth breathing (82.61%), and hearing loss (78.26%).
Epidemiological Aspects of Dysphonia in Tertiary Care Hospital
Objective: To find out the prevalence, etiology, risk factor, presentation and treatment option of the dysphonic people, facilitate the prevention of the risk and predisposing factor of it. Study Design: Cohort retrospective study. Setting: Academic tertiary care medical centre. Subjects and Methods: A total of 1739 dysphonic patient’s demographic data collection and analyzed, in the department of Otolaryngology and Head-Neck Surgery, Comilla Medical College, and Comilla Medical Centre, Bangladesh from 01 July 2016 to 31 June 2019. Results: The incidence of dysphonic patients was1.16%, and the yearly prevalence of 33.33%. Out of 1739, the male was 1006 (57.85%), and the female was 733 (42.15%), 50-59 years were highest presentation 488 (28.06%). Among 1739, non-specific chronic laryngitis was 1015 (58.37%), dysphonia without structural change (MTD) 417 (23.98%), and malignancy 90 (5.17%). Off them, smoker was 911 (52.39%), voice abuser 469 (26.97%), industrial worker was 477 (27.43%), teacher 359 (20.64%), singer 151 (8.68%), and slum dweller was 528 (30.36%).
Perichondritis of the Pinna: A Real Compulsion of Hospital Linger
Background: Perichondritis of pinna is relentless, deadly cellulitis of the auricular cartilage, which may lead to necrosis. As a result, shrinkage and deformity of the pinna may occur. Different etiological factors, including self-trauma by high ear piercing for beauty and fashion, accident, assault, infections, progressive otitis externa, and allergy, are mentionable. Methods: It is a cohort retrospective study of 63 cases in the Department of Otolaryngology and Head-Neck Surgery, Cumilla Medical College, and Cumilla Medical Centre, Bangladesh, from 01 July 2016 to 31 June 2019. Results: The incidence of perichondritis among ENT casualty was 0.86%. Off 63, the male was 33 (52.38%), and the female 30 (47.62%), children were 27 (42.86%), and adult 36 (57.14%), in which lowest age 02 years, highest 76, mean age 21.952, and the standard deviation 16.676, diabetic 08 (12.70%), smoker 09(14.29%), and all were unilateral.
Nasolabial Cyst: A Sporadic Disease Entity
Background: The formation of the maxilla is associated with the fusion of different types of embryological elements. If any primitive ectodermal cells buried in the embryonic fusion, there is a formation of a cyst. Nasolabial or nasoalveolar cyst is one of them. It is a rare and non-odontogenic, soft tissue origin occurs beneath the alar nasal. Methods: It is a cohort retrospective study of 09 cases in the Department of Otolaryngology and Head-Neck Surgery, Cumilla Medical College, and Cumilla Medical Centre, Bangladesh, from 01 July 2016 to 31 June 2019.
Adenoids with Glue Ear: Incidence, Management and Outcome
Background: In the 17th and 18th centuries, Santorini and Wilhem Mayer described the adenoids. Enlarged adenoid or adenoids is a common disorder in children, not only compromise the natural pathway of breathing, but it also obstructs the nasopharyngeal opening of Eustachian tubes. As a result, retention of fluid in the middle ear cavity and the development of glue ear or otitis media with effusion (OME). If it happens, the children present with hearing loss, delayed speech and language, poor social behavior, and may with difficulties of balance. Methods: It is a cohort retrospective study of 251 cases in the Department of Otolaryngology and Head-Neck Surgery, Cumilla Medical College, and Cumilla Medical Centre, Bangladesh, from 01 July 2016 to 31 June 2019. Results: Incidence of adenoids with glue ear, out of total ENT routine operations was 3.54%, and adenoidectomy-tonsillectomy operations were 29.05%. Of them, the male was 102(40.64%), the female was 149(59.36%), 3-5 years were 83(33.07%), 6-10 years 107(42.63%), and 11-15 years 61(24.30%).
Incidence and Outcome of Vocal Cord Polyp: An Endoscopic Experience and Perception
Background: The larynx is an intricate part of the body which has to perform a very composite function like protection, respiration, phonation. To carry out the function, larynx should be adaptable and flexible. Any structural disorder like vocal cord polyp alter these specialized function and squander the laryngeal pacing, and increasing voice discomfort of the patient like hoarseness, dysphonia, and odynophagia day by day. Phonosurgery is the mainstay of treatment. Methods: It is a cohort retrospective study of 54 cases in the Department of Otolaryngology and Head- Neck Surgery, Comilla Medical College, Bangladesh, from 20 February 2017 to 27 October 2019. Result: Incidence of vocal cord polyp in laryngeal disorder patient was 6.83%, and the laryngeal operative patient was 28.27%. All patients were treated by micro laryngeal surgery with rigid Hopkin’s laryngeal telescope, and setback was 2 (3.70%). Of them, male were 41(75.92%), females were 13(24.08%), below 20 years were 2(3.70%), 20-50years 39(72.23%,) and above 50years 13(24.07%), unilateral was 50(92.59%) and 4(7.41%) were bilateral, smoker was 41(75.92%), nonsmoker 13(24.08%), industrial workers were 21(38.89%), professional voice user was 13(24.07%).
Incidence and Outcome of Second Branchial Cleft Anomalies: A Three Years Experience and Appreciation
Background: Second branchial cleft anomalies are remarkable of all other inborn errors of branchial apparatus. As a congenital, it may be presented at birth but usually manifests in early teenagers and young adults. Like all other inborn errors, it is better to correct earlier before suffering from any complications. Methods: It is a cohort retrospective study of 15 cases in the Department of Otolaryngology and Head- Neck Surgery, Cumilla Medical College, and Cumilla Medical Centre, Bangladesh, from 01 July 2016 to 31 June 2019. Results: Incidence of anomalies among routine ENT operations was 0.21%. Of 15, the cyst was 04(26.27%), sinus was 10(66.66%), the fistula was 1(6.67%), children were 09(60%), the adult was 6(40%), and young adult and an early teenager were 07(46.66%), male was 11(75.33%), and females were 04(26.27%), bilateral were 2(13.33%), unilateral was 13(86.67%) in which right was 10(76.33%), and left was 03(23.08%), painful cyst with abscess was 01(6.67%), the painless cyst was 03(20%), sinus with the mucoid fluid discharge was 07(46.68%), painful sinus with mucopurulent discharge and skin-excoriation was 03(20%), painful fistula with mucopurulent discharge and skin-excoriation was 01(6.67%), postoperative wound infection was 2(13.33%). Recurrence and the branchio-oto-renal syndrome were absent in our study.
latrogenic and Idiopathic Vocal Cord Paralysis: A Real Obligation of Endoscopic Appraisal
Background: The larynx is an inevitable mouthpiece of the human body which has to execute very associated assignment like protection, respiration, and phonation. To fulfill this mission, the larynx should be accommodated, integrated, and sensible. The vocal cord is the concern of larynx, which served all-purpose. If any occurrence due to iatrogenic, idiopathic, or any other causes shut down the vocal cord mobility, the patient is facing miserable complexity in their life. Methods: It is a cohort retrospective study of 67 cases in the department of otolaryngology and HeadNeck Surgery, Bangladesh, from 01 July 2016 to 31 June 2019.
